I acknowledge that if I select "FFS Rapid PCR|Antigen Test for Travel/Peace of Mind (No Insurance)" that I do NOT have any symptoms or exposure. I understand that this visit is not covered by insurance and I will be charged. If I have COVID symptoms or exposure, I need to select "Visit 1- Urgent Care" under "Reason for Visit" at the top of this page.

I acknowledge that if I select "Flu Shot" that I am aware the following insurances do not cover flu shots at our center (NHP, BMC, Allways, Network Health, United, Humana, MH PSN & MH HSN). If I have one of these insurances I consent to paying $40 for the shot.
